[Lnc-business] UPDATED POLICY MANUAL

Attached please find the update policy manual.  This includes the
changes passed in 2018-18 and the addition of:

A revision date to the footer
An explanatory footnote to the log of changes that those changes are
substantive changes formally passed

Staff please replace on site.

Please note that I am working on creating a version in which the table
of contents is linked to the TOC or at least make it a PDF bundle that
has an embedded navigation option to go directly to the pages.
