[Lnc-business] Updated Policy Manual

Using the actual date the amendment was approved makes it possible for a
reader to easily find the details of the change in the LNC minutes. There
was either an LNC meeting on that date, or it was an email ballot, in which
case it shows up in the minutes of the subsequent LNC meeting.
My practice was to also use the last-amended date for the document's title
page, though it's not unreasonable to alternatively use the publication
date.
