[Lnc-business] Updated Policy Manual

In the updated version of the Policy Manual, the footers need to be
updated, as they still show the June 30 date.

On page 61, the new entry in the "Log of Changes" shows the change date as
August 31, 2018, which is still a future date.  The email ballot adopting
the change ended on July 27, 2018.
