Dan,
I think for 3 a copy of a voter registration card would show
registration and for 6 I would certainly require the name and contact
information of the dedicated campaign manager. Hopefully it is not the
candidate.
For 4 & 5 I would trust staff's judgement with perhaps a referral to the
Candidate Support Committee if there is any question.
