In debate on an appeal, only the Chair can speak twice (pp 388-389) - we
don't tend to follow this.

And on page 401, it seems to me that it says or at least implies that votes
requiring 2/3 do not start with a voice vote, but rather must at least be a
hand raise or standing (I would love to hear Ms. Mattson's opinion on that).
